What is the Transitions Program?

The Transitions Program is for young adults who are currently experiencing homelessness. The program helps with case management, employment, and education; while offering short-term housing to help youth get on their feet.

Do I qualify for the Transitions Program?

If you are 17 to 24 years old, and literally homeless; you meet the initial program requirements.

How do I know if I’m homeless and without resources?

You meet homeless requirements if you are…

  1. Sleeping/Camping Outside
  2. Staying at a homeless shelter
  3. Sleeping in your Car
  4. Couch Surfing (a new place every few days)
  5. Staying in an abandoned house/apartment

I stay with my family, but it feels like a toxic environment. Can I still qualify?

If you are living with family, you are not considered homeless. If you are struggling in your family’s home – please consider reaching out to a counselor at YST to help with any conflicts that might be making things hard.

How long does it take to get into the program?

We have a limited number of apartments in the Transitions program. The admissions process usually takes 2 to 3 weeks, and if accepted into the program, youth will go on the housing waitlist. It can sometimes take several weeks before apartments are open.

How can I meet basic needs while I’m waiting? (shower, food, clothes, etc.)

Youth can visit The Station, the YST Drop-In Center to meet basic needs during the waiting process. The Station is open Monday-Friday, 11am to 4pm. Youth can go anytime to get a hot meal, take a shower, do laundry, etc. during open hours.
